1. If you have an U.S. Social Security Number (SSN*) you may look up your PID on-line at the following secured site: 
   
   https://itsapps.unc.edu/PIDCreate/jsp/wmp.jsp


2. Contact the PID Office with your full name, gender, date of birth and SSN*. 
   Or, for an individual request you may call the UNC OneCard Office. 


3. For batch PID lookup requests please e-mail the PID Office for instructions.

* Note: The Social Security Number is requested by the institution solely for administrative convenience and record keeping accuracy, and is requested only to provide a personal identifier for the internal records of the institution.
